Job summary

Chambers County Public Hospital District No 1 seeks a Community Health Worker to connect clients with resources, provide health education, and coordinate care across home, clinic, and community settings. You will collaborate with medical, behavioral health, and dental teams to improve overall wellbeing.

Responsibilities include assisting with Medicaid/CHIP/SNAP enrollment, documenting encounters, and advocating for clients.

Qualifications

• CHW certification, or ability to obtain within the first 6 months of employment.
• CAC certification, or ability to obtain within first 6 months of employment.
• Bilingual (Preferred).
• Strong interpersonal and communication skills and the ability to work with clients in a diverse community.
• Knowledge of community agencies and resources.
• Working Knowledge of Microsoft Office and Electronic Health Records (EHR).
• High School Diploma or GED.
• At least one (1) year of applicable experience.

Responsibilities

• Coordinate client referrals to address identified needs.
• Develop personalized support plans for clients.
• Provide client advocacy and education on program benefits.
• Document encounters and maintain electronic records.
• Track client progress and report outcomes.
• Travel to various community locations for outreach.
• Promote awareness of health center services and available assistance.
