Job summary

ChristianaCare is seeking a Community Health Worker for the Hospital Violence Intervention Program. You will build trusting relationships with patients through outreach, support, and follow-up, helping identify health goals and overcome barriers to care.

You’ll connect patients with healthcare services, community programs, and social resources, coordinate with healthcare teams, and document progress as you work in patients’ homes and community settings with occasional travel.

Qualifications

• High school diploma or GED.
• 1–2 years of experience in community outreach, healthcare support, social services, or related field.
• Strong communication, relationship-building, organizational, and problem-solving skills.
• Valid driver’s license and access to a reliable personal vehicle.

Ability to work independently and collaboratively with patients, healthcare professionals, and community partners. Valid driver’s license and access to a reliable personal vehicle.

Preferred Qualifications

Associate or bachelor’s degree in Public Health, Social Work, Health Education, or a related field. Community Health Worker Certification. Experience working with underserved or diverse populations. Bilingual communication skills.

Work Schedule & Location

This is a full-time, community-based position in New Castle County with occasional travel to Kent/Sussex Counties. You’ll work with patients in their homes, community locations, and ChristianaCare clinical settings. Flexibility to work occasional evenings and weekends is required.

Hourly Pay Range: $26.03 - $39.05. This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
