Job summary

Wellcentric DC is seeking a Community Health Worker in Baltimore, MD to serve as a liaison among enrollees, the healthcare system, and community resources. The CHW will conduct outreach, home visits, and education to promote preventive care and reduce barriers to access.

The role collaborates with Case Management to support service access, navigate SDoH needs, and enhance member experience. Strong English communication and confidentiality are essential.

Qualifications

• High School Diploma or GED is required.
• Experience with community organizations, verified by references.
• Driver’s license with clean driving record.
• Ability to read, write, and communicate in English.
• Ability to maintain patient confidentiality and privacy.

Responsibilities

• Serve as liaison between enrollees, the healthcare system, and community resources.
• Conduct outreach and home/community visits to engage members and reduce barriers to care.
• Educate and advocate through individual and group interactions to promote preventive health.
• Collaborate with Case Management to support access to services and resource navigation.
• Document outreach, interactions, referrals, and activities in required systems.
