Sinai Chicago is seeking a Community Health Worker I to provide health education, disease management education, and navigation services to patients and community residents under supervision.

The role emphasizes one-on-one and group education, case management under supervision, and building trust within Chicago's West Side and South Side communities, with a commitment to health equity and flexibility in hours.

Qualifications

• High school diploma or GED is required.
• One year of health education, health care, or related experience preferred.
• Ability to provide health education, one-on-one and in groups.
• Familiarity with Chicago's West Side and/or South Side communities preferred.
• Commitment to racial and health equity.
• Punctual, reliable, and willing to learn.
• Valid driver’s license and own car preferred (required for some positions).
• Bilingual in Spanish may be required for some positions.

Responsibilities

• Provide health education, disease management education, and navigation services to patients/community residents.
• Provide case management under supervision.
• Assist with health care and social services navigation.
• Hours are flexible and may include evenings and weekends scheduled in advance.
