Job summary

Memorial Hermann Health System seeks a Community Eligibility Specialist to screen NMDOH needs and navigate clients to resources and medical homes. You will guide SNAP, Medicaid, utility assistance, and other benefits, ensuring accurate documentation and timely enrollment.

You will engage with patients at Community Resource Centers across Greater Houston, supporting follow-ups and coordinating with care teams.

Qualifications

• Education: High school diploma or GED required.
• Two (2) years' experience in healthcare, social services, or community outreach settings preferred.
• Bilingual (Spanish) preferred.
• Knowledge of NMDOH and an understanding of community resources.
• Strong organizational and time management skills, with the ability to manage multiple referrals and follow-ups.
• Comfort using digital platforms and technology to support remote engagement and documentation.
• Ability to work collaboratively as part of a multidisciplinary team while maintaining empathy, discretion, and professionalism.
• Experience using Microsoft Suite (Word, Excel).

Responsibilities

• Conduct screenings to identify NMDOH and document patients’ risk levels across domains (housing, food, utilities).
• Guide clients through the full application process for SNAP, Medicaid, utility assistance, and other benefits.
• Act as resource navigator and educator on scope of services and enrollment steps.
• Follow up with clients and agencies to confirm status and ensure enrollment and access to services.
• Identify trends and barriers to improve access and streamline enrollment processes.
• Collaborate with Memorial Hermann care teams to provide medical and non-medical support.
• Support at other locations to meet patient and program needs.
• Adhere to policies, maintain budget and quality of service.
