Job summary

Durham County Government is seeking a Community Health Worker for the Chronic Care Initiative to bridge clients, families, and providers with Durham’s Public Health services.

The role focuses on connecting residents with health information, assisting in enrollment for community resources, and promoting self-management in chronic conditions, including diabetes prevention, with potential evening activities.

Qualifications

• Requires High School graduation or GED supplemented by two (2) years of experience in social work, health services, counseling, or related work.
• Experience working with diverse communities and community organizations is preferred.
• Excellent written and oral communication skills and ability to work independently and in a team.

Responsibilities

• Serve as a front-line public health staff member in the community.
• Connect residents with health information and services, assisting navigation of resources.
• Promote wellness through culturally appropriate health education.
• Conduct home-based patient assessments on a regular basis.
• Recruit new participants and enroll them in the program via community events.
• Document client encounters in the electronic health system.
• Participate in community events to increase knowledge of resources.
• Assist with gathering documents for program enrollment.
