Job summary

Reporting to the Navigator Program Manager, you will collect data, document case details, and help design individualized care plans while coordinating with a broad network of community partners and healthcare providers.

Qualifications

• Valid Massachusetts driver’s license (or ability to obtain

Responsibilities

• Identify at-risk individuals, enroll eligible participants, collect data.
• Coordinate with community partners to forge durable relationships.
• Assist in coordinating community human services and related resources.
• Develop treatment plans to connect clients with services.
• Enter program data into required systems (e.g., Epic).
• Attend CRN program meetings and agency committees as required.
