Responsabilités

• Provides care coordination and connects patients to social services.
• Performs initial biopsychosocial assessments and conducts home or clinic visits.
• Educates patients on advance directives and POA.
• Develops care plans addressing social needs and health determinants.
• Coordinates and documents client encounters and service plans.
• Travel to clinics or patient homes as needed.
