Job summary

Trinity Health Of New England in Agawam, MA is seeking a Community Health Worker to support care management in our Clinically Integrated Network. You will help coordinate patient services, mail educational materials, schedule appointments, and connect patients with community resources.

Qualifications

• High school diploma required.
• Medical Assistant Certificate preferred; Associate's degree or equivalent medical experience preferred.
• Certified Application Counselor for MassHealth program or willing/able to complete required trainings.

Responsibilities

• Mail educational materials and letters, schedule patient appointments, locate and obtain outside patient records.
• Collaborate with community partners, such as VNA agencies, caregiver programs, and DME providers.
• Use general knowledge of medical conditions, nutrition, and diet to educate patients.
• Assist in locating and updating patient clinical records.
• Facilitate patient and caregiver access to community resources programs, Meals on Wheels, senior centers, chore services, etc.
• Screen patients to assess for depression, substance use, and mental health disorders; respond with appropriate interventions and/or referrals.
