Grande Ronde Hospital is hiring a Community Health Worker (CHW) to support patient care by coordinating communication between patients, providers, clinic staff, and community resources. The CHW identifies barriers to care and assists patients with navigation to services, documenting follow-ups to ensure timely access.

The position is on-site in La Grande, OR, full-time with a pay range of $21.75 to $29.98 per hour DOE.

Competenze

• High school diploma or GED required; bachelor’s degree preferred.
• CHW certification required within 1 year of hire.
• One year of experience in community outreach, health education, social services, healthcare, or related field.
• Current American Heart Association BLS card within six months of hire.
• Current Oregon driver’s license.

Mansioni

• Serve as a communication bridge between patients, providers, and clinic staff through in-office, phone, and home visit interactions.
• Identify patient needs and connect individuals and families to community resources and support services.
• Assist patients with care coordination activities, including referrals, transportation, paperwork, and appointment navigation.
• Track and document patient interactions, referrals, and outcomes in designated systems for reporting and follow-up.
• Maintain knowledge of patient-centered care programs, registries, and reporting requirements.
• Support patient engagement by fostering trust, advocacy, and informed decision-making.
• Collaborate with clinic staff and community agencies to address healthcare and social service needs.
• Participate in meetings, trainings, and maintain CHW certification and competency.
• Uphold patient confidentiality, safety standards, and compliance with policies and regulations.
