Job summary

BEHAVIORAL HEALTH SERVICES INC in Gardena, CA is seeking a Community Health Worker to serve as a liaison among patients, families, healthcare providers, and community resources within an FQHC and behavioral health setting.

The CHW will support patient engagement, health education, care coordination, outreach, navigation, and social support services to improve health outcomes and reduce barriers to care. The role promotes trauma-informed, culturally responsive, and equitable care.

Qualifications

• High school diploma or equivalent required.
• CHW certification preferred or willingness to obtain certification.
• Experience in community health, behavioral health, or related field preferred.

Responsibilities

• Provide outreach, engagement, education, advocacy, and support services to patients and community members.
• Assist patients with navigation of healthcare systems, referrals, social services, and resources.
• Support care coordination activities including scheduling, referrals, and transportation.
• Conduct screenings, intake, assessments, and patient interviews as assigned.
• Deliver health education related to preventive care, chronic disease management, and behavioral health.
• Assist with enrollment, registrations, and accessing healthcare services.
• Document encounters and referrals in the EHR and tracking systems.
• Identify social determinants of health barriers and assist with resource linkage.
• Collaborate with care teams and community partners to support patient plans.
• Participate in community outreach events and health promotion activities.
