Job summary

The Family Practice & Counseling Services Network is seeking a Community Health Worker for the Health Annex in Philadelphia, PA. This full-time role supports PHA CARES-funded outreach, education, and patient navigation to connect residents with medical and social services.

The CHW conducts group and individual health promotion activities, helps residents access care, and participates in health events and community engagement at the Health Annex site.

Qualifications

• High School diploma or GED required.
• 2 years working with people in the health field preferred.
• Familiarity with Philadelphia Housing Authority communities or Housing Choice Voucher residents preferred.
• Certified Health Worker (CHW Certification) preferred.
• Current CPR Certification is required.

Responsibilities

• Assist patients in making and keeping appointments and screenings.
• Motivate patients to engage in health management plans, including exercise and diet.
• Coach patients in managing chronic conditions and understanding care plans.
• Accompany residents to appointments and assist in communicating with health care providers.
• Contact clients to ensure required actions are completed and connect to resources.
• Lead and participate in health promotion and outreach events for PHA CARES.

Education

High School diploma or GED

2 years working with people in the health field
