Crescent Community Health Center in Dubuque, IA is seeking a Licensed Practical Nurse to provide high-quality, patient-centered care within our community health setting. You will administer medications, monitor health, and collaborate with the healthcare team to develop individualized care plans.

The ideal candidate has completion of an accredited LPN program, a current LPN license, and 1–2 years of direct patient care experience, with strong communication and documentation skills.

Qualifications

• Completion of an accredited LPN program.
• Current LPN license in the state of practice.
• 1–2 years of experience as an LPN in a healthcare setting.
• Experience in direct patient care and managing patient records.

Responsabilités

• Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and protocols.
• Monitor patient health with regular assessments, vitals, and condition changes.
• Assist in developing and implementing individualized patient care plans.
• Provide education to patients and families on health maintenance, disease prevention and treatment options.
• Collaborate with physicians, RNs, and other healthcare professionals to ensure cohesive care.
• Maintain accurate patient records and document changes in condition.
• Participate in quality improvement to enhance patient care and operations.
• Ensure compliance with healthcare regulations, policies, and procedures.
