  • Resource Navigation: Connect AHA PBRA residents and their families with health and social resources, supporting them in taking an active role in managing their health.
  • Data & Systems Support: Assist in creating and maintaining online systems for tracking referrals, resident engagement, and social service utilization.
  • Health Education: Deliver culturally responsive health education and support initiatives aimed at reducing disparities in the AHA community.
  • Community Engagement: Support community health initiatives such as SDoH screenings, vaccine outreach, health promotion events, and service referrals to enhance community resilience and reduce chronic disease risks.
  • General Support: Carry out additional duties as assigned to gain broad exposure to the field of community health work.

  • Excellent communication and presentation abilities.
  • Proven ability to prioritize tasks, meet deadlines, and maintain punctuality.
  • Demonstrated comfort working with diverse populations, including public housing residents and partner agencies.
  • Valid Texas driver's license, clean driving record, and access to a personal vehicle.
  • Bondable.
Background Check Requirements
  • A Criminal Background Check including a sex offender registry check. A Driving Record, Texas Driver's License, and a Physical/Drug Screening will be required upon offer of employment.
