Job summary

Integrated Services for Behavioral Health is seeking a CHW Field Placement Intern to support community health outreach and education under supervision.

You will work with clients and families across the life span, helping identify needs, connect to resources, and support non-clinical activities and events in the community.

Qualifications

• Recently completed CHW certificate program is REQUIRED.
• Interest in community health and health equity.
• Strong communication, empathy, and cultural competence.

Responsibilities

• Assist in outreach to individuals and families to identify health and social service needs
• Serve as liaison between community members and health organizations
• Educate clients about health topics, preventive care, resources, and referrals
• Coordinate, schedule, and track client progress
• Participate in team meetings, training, and supervision sessions
• Support non-clinical tasks such as health promotion events and community engagement

The CHW Intern will support community health outreach, education, and other supportive services under the supervision of program staff, working with clients and families in the community across the life span.

Primary Responsibilities

• Assist in conducting outreach to individuals and families to identify health and social service needs
• Serve as a liaison between community members and health/social service organizations
• Educate clients about health topics, preventive care, resources, and referrals
• Help with coordination, scheduling, and tracking client progress
• Participate in team meetings, training, and supervision sessions
• Support non-clinical tasks (e.g. health promotion events, workshops, community engagement)

Qualifications / Ideal Candidate

• Recently completed CHW certificate program- REQUIRED
• Interest in community health, health equity, social determinants of health
• Strong communication, empathy, and cultural competency
