Job summary

Pacific Health Group is hiring Street Outreach Advisors to connect community members with care and support services in a hybrid field-based role across local counties. You will spend significant time in the community, building partnerships and coordinating referrals to Enhanced Care Management and related services.

A strong emphasis on relationship-building and reliable CRM documentation is required. The position requires driving in California, traveling within the county, and meeting enrollment

Qualifications

• Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work.
• Comfort working independently in a fast-paced, community-facing environment.
• Ability to manage outreach activity, relationships, and follow-through across multiple partners.

Responsibilities

• Identify and enroll referrals by building partnerships with community organizations.
• Engage individuals and expand access to PHG services through field outreach.
• Document outreach activity and referrals in CRM, ensuring follow-through with internal teams.
