Job summary

Pacific Health Group is seeking Street Outreach Advisors to work in the community, bridging clients to Enhanced Care Management and related services. This field-based role emphasizes in-person outreach, strong partnerships, and referral pathways in high-need areas around Redding in a hybrid model.

You will spend most weeks in the field meeting partners, documenting activity in CRM tools, and coordinating with internal teams to close referrals.

Qualifications

• Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work.
• Ability to work independently in a fast-paced, community-facing environment.
• Strong organizational skills and comfort using mobile tools and CRM systems.
• Driver's license and ability to travel locally.
• Willingness to work occasional evenings or weekends.

Responsibilities

• Identify and prioritize partner organizations across local markets.
• Build and maintain referral-generating partnerships for PHG services.
• Own a outreach territory by developing and sustaining partner relationships.
• Spend majority of week in the field meeting partners and engaging the community.
• Identify individuals who may benefit from PHG services and facilitate warm referrals.
• Document outreach activity and referrals in CRM.
• Coordinate with internal care teams to track referrals and close-loop.
• Share field observations to inform outreach strategy.
• Ensure consistent referral activity and growth in territory.
• Establish strong community partnerships across priority service areas.
