Job summary

Pacific Health Group is seeking a Street Outreach Advisor to connect individuals with care and support services in high-need communities. The role is highly field-based, with partners and individuals met in the field to expand access to services across behavioral health, housing, and related supports.

You will spend time in the community, build referrals, and collaborate with internal teams to track outcomes and improve outreach strategies.

Qualifications

• Experience in community outreach or public health field-based work.
• Willingness to travel locally and some evenings/weekends.

Responsibilities

• Identify and prioritize community-based organizations and develop referral partnerships.
• Engage individuals in field outreach and facilitate referrals or enrollments.
• Document outreach activity and referrals in CRM and coordinate with internal care teams.
