Provides health education outreach program activities for large participant groups in identified counties throughout Tennessee and surrounding areas to help maximize and maintain healthy lifestyles. A Brief Overview

Provides health education outreach program activities for large participant groups in identified counties throughout Tennessee and surrounding areas to help maximize and maintain healthy lifestyles. Responsible for the management of challenging group dynamics in order to assure proper program delivery. Serves as a resource to assist individuals, other healthcare workers and the community. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

What you will do
  • Presents health information to patients, families, or community groups in order to promote healthy lifestyle decisions and chronic disease management.
  • Provides health education and awareness to the assigned community program.
  • Responsible for large group participant management, especially groups with behavioral issues and/or special needs.
  • Enters data, documents and evaluates the effectiveness of program activities.

  • Bachelor's Degree Health Services

Work Experience Requirements
  • 1-3 years Customer service, collections or call center

Knowledge, Skills and Abilities
  • Proficiency in the Microsoft Office program or programs as indicated in the specific function within the department.
  • Ability to operate standard office equipment.
  • Ability to understand and compose complex written materials.
  • Ability to exercise independent judgment with tact and diplomacy in all situations.
  • Must be able to assess needs of participants and their families and respond appropriately.
  • Strong ability to integrate knowledge and theory into practice.
  • Ability to demonstrate skill in organization, coordination and time management.
  • Must be able to provide effective instructions to participants.
  • Must be able to communicate verbally in an effective and positive manner with participants and their families.
  • Must be able to demonstrate effective communication and negotiating skills with and between individuals and groups of people from all socioeconomic backgrounds, including Associates, social services providers, business entities, and students and their families. Strong influencing, coaching and consultative skills.

Supervision Provided by this Position
  • There are no supervisory or lead responsibilities assigned to this position.

Physical Demands
  • The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
  • Must have good balance and coordination.
  • The physical requirements of this position are: light work - exerting up to 25 lbs. of force occasionally and/or up to 10 lbs. of force frequently.
  • The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
  • Ability to react quickly to emergency situations.
  • Frequent travel outside office on a regular basis.
  • Exposure to patient bodily fluids as well as exposure to hazardous or poisonous materials.
