Job summary

Sea Mar Community Health Centers, a Federally Qualified Health Center (FQHC) founded in 1978, is seeking a Health Educator I to provide quality health education to patients and in-service education to staff. This role also builds relationships with community groups to improve access to services and ensures current educational materials.

The Health Educator I works with clinic staff and physicians to emphasize preventive care, meets productivity goals, and helps strengthen Sea Mar's mission

Qualifications

• Provides quality health education services to Sea Mar patients.
• Delivers in-service education to Sea Mar staff.
• Initiates and maintains contact with community groups to increase accessibility of services.
• Ensures information and materials are current and accurate.

Responsibilities

• Assess health education needs of target population and provide education on hypertension, diabetes, and related topics.
• Design, promote, present and evaluate community health sessions.
• Develop collaborative relationships with local health, educational and social service providers.
• Attend staff meetings and in-service trainings as scheduled.
• Complete and submit weekly/monthly reports.
• Review and update health education materials and participate in Literacy Review.
• May provide blood pressure screening for residents in care settings.
• Chart notes in Electronic Health Records (EHR).
