Job summary

ChristianaCare is seeking a Health Coach to support patients in chronic disease prevention and self-management in Wilmington, Delaware. You will apply health behavior change theory, conduct coaching sessions, and collaborate with clinicians to reinforce care plans.

As part of an interdisciplinary team, you will design coaching curricula, track outcomes, and promote engagement in programs across New Castle, Kent, and Sussex Counties. A bachelor's degree and NBC-HWC/CHES preferred; travel required.

Qualifications

• Bachelor's degree in health promotion, public health, health education, psychology, nutrition, nursing, or related field.
• Minimum two years of health coaching or behavior-change support experience; chronic disease experience preferred.
• NBC-HWC and/or CHES credentials preferred; familiarity with healthcare settings a plus.

Responsibilities

• Apply health behavior change and self-management knowledge to assess participant needs with professional judgment.
• Provide individual and group coaching using participant-centered approaches.
• Use motivational interviewing and goal setting to support autonomy and behavior change.
• Collaborate with clinicians to reinforce care plans and coordinate referrals as needed.
• Develop and adapt coaching curricula and materials based on participant needs and literacy.
• Document encounters and outcomes in designated systems and track program metrics.
