Job summary

Lifelong is a community health organization in Seattle dedicated to removing barriers to health with compassion. The role focuses on improving health outcomes for clients with chronic and behavioral health needs through comprehensive home assessments, care planning, and resource connections.

Responsibilities include caseload management, Medicaid-related processes, and advocating for client benefits while maintaining HIPAA-compliant records and collaborating with partner agencies.

Qualifications

• Relevant social services experience for high-barrier clients.
• Valid driver’s license and reliable transportation for on-site visits.
• One of: Master’s degree + social service experience; Bachelor’s + 1 year; Any Bachelor’s + 2 years; High school + bilingual + 3 years; High school + 4 years; Associate + 2 years with DSHS.

Responsibilities

• Conduct home assessments and review medical information for each client.
• Manage caseload and develop care plans; document activities and maintain accurate records.
• Review financial eligibility and coordinate with healthcare, mental health providers, nursing staff, and others.
• Connect clients and caregivers to needed services such as substance use treatment, mental health, and medical care.
• Advocate with agencies to help clients receive appropriate benefits or services.
• Assist with budgeting and payment authorizations using relevant state portals.
