Job summary

Responsibilities

• Interview and assess participant needs through interviews with participants referred by OD R and other referrals.
• Verify and document the eligibility of participants.
• Develop a service plan with the participant to address needs and goals.
• Assist with public benefits advocacy and linkage.
• Work with faculty to assist with placement and case management upon release.
• Collaborate with area agencies to coordinate efforts for participants.
• Attend inter-agency meetings and coalitions to support participants.
• Maintain concise case records and provide daily/monthly statistical information.
