Job summary

CommUnityCare Health Centers is seeking a Community Health Worker to provide culturally appropriate health education and assistance to Austin-area community members, helping them access needed care and navigate the health system.

The role involves collaborating with the care team, acting as a cultural mediator, and empowering patients and families in care management. A high school diploma and CHW certification or equivalent training are required.

Qualifications

• High school diploma or equivalent.
• One year of experience in a community health setting or related role.
• Certified Community Health Worker (CHW) or completion of a 160-hour CHW training within 6 months.
• Valid Texas Driver’s License and access to a personal vehicle.
• Proof of automobile liability insurance as required.

Responsibilities

• Collaborate with care team to provide patient-centered care.
• Provide cultural mediation between community members and CommUnityCare.
• Empower patients and families to engage in care management.
• Help patients set goals and attend appointments.
• Coach patients in managing health conditions and self-care.
• Identify patient needs for referrals to social services and community resources.
• Serve as liaison between patients and healthcare systems and assist as needed.
• Ensure patients follow through with provider plans and attend follow-ups.
