• Bachelor’s degree in Social Work, Public Health, Psychology, or related field preferred.

• Identifies barriers like housing, food insecurity, and transportation and links to resources.
• May conduct outreach, home visits, and participate in community health events.
• Maintains records to support advocacy and program evaluation.
• May assist with health education and basic screenings under supervision.
