Job summary

Lifekind Health is seeking a Community Health Worker in Rancho Mirage, CA. This full-time role supports Members by navigating healthcare systems, identifying barriers to care, and delivering person-centered health information.

The position emphasizes collaboration with a transdisciplinary team, motivational interviewing, and community advocacy to improve outcomes for a complex patient population.

Qualifications

• High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.
• Valid California Driver’s license and valid automobile insurance required
• Ability to work with a complex patient population.
• Ability to work independently and be self-directed.

Responsibilities

• Communicates to clients/patients the purposes of the program and helps identify socio-economic issues affecting health and develop plans.
• Assists Members in navigating the healthcare system by phone and in person to overcome barriers to care.
• Engages with Members using evidence-based approaches (such as Motivational Interviewing) to promote collaboration with the medical/behavioral team.
• Provides information on IEHP Member benefits and services and advocates for Members in home and community settings.
• Builds and maintains positive working relationships with Providers and other team members.
