Responsibilities

• Documents patient encounters and maintains electronic files, reports, and assessments.
• Educates clients on proper use of health services and assists with care plans.
• Coordinates with care teams and community organizations to connect patients to resources.
• Provides care, follow up, and education in home, clinic, or community settings.
