Job summary

Trinity Health seeks a Community Health Worker to provide tailored health education, social support, and connections to community resources, focusing on reducing health disparities. You will deliver programs to prevent and improve chronic disease management using a structured protocol for assessments and education.

You will collaborate with clinical and community partners, perform assessments, referrals, and support care coordination with electronic health records.

Qualifications

• High school diploma or GED and at least one year of community involvement experience.
• Knowledge of lived experiences and cultural background of patient communities.
• Must complete CHW training/certification within 6 months of employment.
• Must be comfortable operating in a collaborative, shared leadership environment.
• Personal integrity, honesty and caring; ability to promote Trinity Health values.

Responsibilities

• Provide high-value services to patients with complex social and health needs in clinical and community settings.
• Conduct assessments, identify barriers, and connect patients to resources and services.
• Use EHR and program protocols for care coordination and documentation.
• Communicate effectively with patients, families, and providers to improve care coordination.
• Support ongoing professional development and participate in performance improvement activities.
