Job summary

MaineHealth in Saco, ME seeks a Community Health Worker to join the Lend a Hand care management program. You will help patients access resources, support chronic disease self-management, and coordinate culturally appropriate care in a primary care setting.

The role runs about 40 hours weekly, with a Day shift and a hybrid schedule (up to 2 days remote after training). Experience in healthcare management is advantageous for success.

Qualifications

• Education: High school diploma or GED preferred.
• License: Valid Driver’s license and clean driving record if driving is required.
• Experience: Two years in social services or health care preferred.
• Must be a resident of the community served with good local knowledge.
• Basic computer skills preferred.

Responsibilities

• Assist patients in accessing resources and services.
• Support disease self-management and care coordination.
• Provide culturally appropriate care and services within primary care.
• Engage with patients over a ~6-month period as part of care management.
