Ingham County Health Department seeks a Community Health Worker to connect underserved communities with health and social services. Through home visiting, you help clients overcome barriers to care, education and employment, coordinating with providers and resources.

Key responsibilities include form completion support, outreach to distribute resources, document management in client systems, SDOH assessments, referrals for Medicaid, and navigation across programs.

Qualifications

• High School diploma or equivalent required; MiCHWA curriculum preferred.
• Experience in social services, public health, or maternal child health preferred.
• Interest in community health and serving diverse populations.

Responsabilités

• Assists clients with filling out forms and verifies proper completion.
• Coordinates with clients and providers for appointments in person or by phone.
• Maintains program client system documentation for care coordination.
• Assesses social determinants of health and provides referrals.
• Conducts outreach and distributes resource guides to the community.
• Supports Medicaid and other program applications; navigates state data systems.
