• Bachelor's or Associate degree preferred in Human Services, Psychology, Nursing, or related field.

Responsibilities

• Serve as ambassador for CICOA’s Mission, Vision and Values.
• Provide exceptional customer service and care coordination with health professionals.
• Conduct home visits to support older adults and individuals with disabilities.
• Document accurately and timely in program systems.
• Collaborate with team to reduce barriers to health care access.
• Support outreach, referrals, and follow-up with clients.
