Job summary

Eskenazi Health in Indianapolis is seeking a Community Health Worker for Primary Care. The CHW will work with patients at the health center and in the community to improve understanding and adherence to treatment plans, assess social determinants of health, and help patients set and reach self-management goals.

Qualifications

• High school diploma or GED with 2 years of community related experience
• Associate's degree preferred
• Community Health Worker certification preferred
• Flexible hours, sometimes working on weeknights and weekends
• Valid Indiana driver's license that meets Eskenazi Health's driving policy requirements

Responsibilities

• Advocate, Care, Teach and Serve with emphasis on the vulnerable Marion County population; model Eskenazi's values
• Document health, social and environmental assessments timely, accurately and with good quality, identifying barriers to accessing appropriate healthcare or other concerns within the patient's home environment
• Perform home visits to patients to provide support, encouragement, and guidance
• Develop supportive and therapeutic relationships with patients and their families
• Document home/telephone visit outcomes in a timely manner
• Utilize motivational interviewing to encourage patients to modify their behaviors
• Recommend community resources for housing, food, clothing, transportation and utilities
• Carry a caseload of at least 100 patients
• Advocate for patients to ensure access to needed services and develop community resources
• Identify high risk symptoms or circumstances and notify higher level clinical staff for consultation
• Work with family and community supports to meet goals
• Adjust activities to reflect appropriate care for patient age
• Provide quality case management and training to patients
• Monitor patient progress and document follow-up
• Participate in clinical supervision

Education

High School Diploma or GED

Associate's degree preferred

Community Health Worker certification preferred
