Job summary

Northwell Health in New York is seeking a Community Health Worker liaison to grant support to vulnerable community members, conduct assessments, and perform in-home visits while coordinating referrals to social services. You will be the main point of contact, supporting care activities and empowering clients to manage their health.

The role focuses on health education, outreach, service coordination, and advocacy, including data entry and acting as a resource for both CHWs and members.

Qualifications

• High School Diploma or equivalent required.
• 1-3 years of relevant experience, required.

Responsibilities

• Serves as a mentor for Community Health Workers.
• Provides oversight in care of members to improve health outcomes and promote well-being.
• Acts as a resource for CHW and members.
• Provides support and basic health and education about material and general health care and community resources.
• Conducts one-on-one health education, community outreach, regular in-home visits and service coordination.
• Provides various supports, services, entitlements and benefits for members.
• Supports members in developing health management care plans.
• Empowers members by promoting effective approaches, problem solving and lifestyle changes.
• Works to establish positive and support relationships and effective works with a diverse group of members.
• Delivers coaching and mentorship on chronic health conditions and self-care.
• Enters client data into software on a daily basis.
• Advocates for members and their families and collaborates with the care team as needed.
• Perform all related duties, as required.
