Job summary

You will work to empower patients to understand their diagnoses, manage chronic conditions, and access community resources.

The role focuses on building bridges between patients and the healthcare system while documenting care and participating in department initiatives.

Qualifications

• Knowledge of community health, public health, and outreach principles.
• Verbal and written communication aptitude is required.
• Current CHA CHW certification issued by the Oregon Health Authority is mandatory.

Responsibilities

• Act as a bridge between patients, their support networks, and the healthcare system.
• Educate and empower patients to understand diagnoses, manage chronic conditions, and build self-care skills.
• Connect patients to social resources (housing, food, utilities) via regional Service Integration Teams.
• Educate patients about healthcare topics and available community programs and tools.
• Document services accurately in the patient health information record.
