Job summary

CICOA Community Health Worker in Indianapolis, IN, full-time position supporting clients through education, goal setting, and care coordination. You will work with Care Managers, Supervisors, and Directors to improve health outcomes and self-sustainability, including home visits and community outreach.

Responsibilities include reducing barriers to healthcare, delivering health education, maintaining documentation, and providing compassionate support.

Qualifications

• High school diploma or equivalent required; CHW certification or willingness to complete training within six months.
• Two years of relevant experience preferred in medical, care management, or Medicaid/Medicare settings.
• Bachelor's or Associate degree in human services, psychology, nursing, or related field preferred.
• Experience working with older adults and state Medicaid processes preferred.

Responsibilities

• Provide person-centered goal setting, education, and support.
• Perform care transitions and coordinate with Care Managers, Supervisors, and Directors.
• Conduct home visits and community outreach to improve health outcomes.
• Maintain accurate documentation and data entry in the program system.
• Protect privacy and confidentiality per HIPAA.
