Summary

As a community Habilitation DSP you are matched to an individual based on likes, location and availability. Generally you will be going out into the community with the individual. This position requires a great amount of commitment, self‑motivation and solid communication skills.

Essential Duties and Responsibilities

The work is usually one‑to‑one.

• Participates in a person‑centered approach to program plan development and participates in activities to build competencies and integrate individuals into their communities.
• Assists individuals to identify social, recreation and other community resources they may wish to access and devises plans to achieve that access.
• Negotiates schedules and daily activities directly with the individual and/or caregiver; establishes and maintains regular hours with individual(s) that you work with.
• Completes all mandated reporting requirements by Wildwood and regulating agencies.

Other duties may be assigned.
