The Direct Support Professional - Community Habilitation and Respite (DSP-CH/R) assists people with intellectual and developmental disabilities (IDD) develop and maintain the skills needed to care for themselves and become involved, contributing members of their communities.

DETAILED RESPONSIBILITIES

• Assist with acquiring, retaining, and improving self-care and independent living skills at home and in the community
• Promote self-determination, self-advocacy, and making informed choices
• Promote community inclusion, including door-to-door transportation to and from activities
• Take all reasonable steps to ensure the health and safety of the person supported
• Participate in bi-annual team meetings to review service plan
• Complete daily and monthly service documentation as required by EOC, OPWDD, and Medicaid

QUALIFICATIONS

• Proficiency in using technology, including mobile applications and computer-based systems.
• Excellent organizational skills with a high level of accuracy and attention to detail.
• Demonstrated cultural awareness and the ability to work effectively with individuals from diverse backgrounds.
• Strong verbal and written communication skills, with the ability to understand and follow instructions effectively.
• Must successfully complete all required background screenings, including fingerprinting and clearance through the New York State Justice Center.

EDUCATION/TRAINING/ EXPERIENCE

• High school diploma or equivalent (minimum requirement)
• Associate's degree or higher in the health and human services field, preferred.
• Have one year of experience working with people with developmental disabilities.

REQUIREMENTS

• A Valid Driver's License and Insured Vehicle.
• PPD (TB test required with proof of results; no more than 1 year old)
• Availability to work during daytime hours on Saturdays is required
