Job summary

Open Cities Health Center in Saint Paul, MN, seeks a Primary Care Physician to deliver holistic clinical care to a diverse patient population, considering social determinants of health, and coordinating with mental health, case management, eye, chiropractic, and dental teams.

The physician will participate in QA/QI, maintain timely EHR documentation, mentor students, and uphold HIPAA and clinic policies while striving to meet FQHC production targets.

Qualifications

• MD or DO degree from an accredited medical school.
• Board-certified in Family Practice, Internal Medicine, Pediatrics, or Psychiatry.
• Current State of Minnesota medical license.
• Current DEA License.
• Demonstrated competence working with racially, culturally, and economically diverse patients.

Responsibilities

• Provides direct clinical care to patients.
• Coordinates care with mental health, case management, eye, chiropractic, and dentistry teams.
• Develops patient care plans including diagnostics, treatments, and follow-up.
• Maintains timely EHR charting and documentation.
• Participates in QA/QI initiatives and provider team meetings.
• Mentors students, trainees, and clinical staff.
• Adheres to HIPAA policies and clinic procedures.
