Job summary

Open Cities Health Center in Saint Paul, MN, is seeking a Primary Care Physician to provide holistic clinical care to patients in a wrap-around service environment.

You will work with diverse patients, develop care plans, and collaborate with mental health, case management, eye, dentistry teams; daily duties include QA/QI participation and mentoring students. MD/DO and board certification required.

Qualifications

• MD or DO degree from an accredited medical school.
• Board certification in Family Practice, Internal Medicine, Pediatrics, or Psychiatry.
• Current Minnesota medical license and an active DEA license.
• Proven ability to work with racially, culturally and economically diverse patients.

Responsibilities

• Provide direct clinical care to patients.
• Maintain accurate EHR documentation in a timely fashion.
• Develop patient care plans including testing, treatments, and follow-ups.
• Refer patients to mental health, case management, eye, dentistry, and other teams as needed.
• Participate in QA/QI initiatives and provider team meetings.
• Mentor students, trainees and clinical staff.
• Adhere to HIPAA and organizational policies.
