Job summary

Sacramento Native American Health Center, Inc. is hiring a Medical Case Manager (CMC) to deliver comprehensive case management for patients with complex physical, behavioral, dental, and social needs.

You will manage referrals, document care plans, and collaborate with healthcare providers and social services to ensure coordinated care. The role involves patient engagement, data collection, and preparation of shared care plans, with occasional float to other SNAHC locations as needed.

Qualifications

• Two years or more in a Health, Social Services, or related field.
• Experience in an Electronic Health Record (EHR) environment.
• Experience with diverse populations, including homeless individuals and low-income families.

Responsibilities

• Coordinate referrals based on eligibility and provider assignment.
• Assist with DME referrals for the medical department.
• Maintain productive engagement with patients through in-person meetings and outreach.
• Develop individualized care plans with short- and long-term goals.
• Document patient interactions and care plans per program requirements.
• Collaborate across departments for care coordination and meetings.
