Job summary

Opportunity Partners in Minnesota seeks an Employment Specialist II to coordinate and support people with disabilities in achieving community-based employment. You will help clients develop skills and independence through individualized planning and hands-on training.

Responsibilities include intake, case management, developing support plans, and marketing services to employers. A valid driver’s license is required, with stated education/work experience qualifications aligned to nonprofit

Qualifications

• Bachelor's degree in a field related to human services, education, or health plus one year of full-time work experience.
• Associate degree in a field related to human services, education, or health plus two years of full-time work experience.
• Diploma in a field related to human services, education, or health plus three years of full-time work experience.
• Minimum 50 hours of education/training related to human services, education, or health and four years of full-time work experience, supervised by qualified staff.

Responsibilities

• Assist individuals with securing community-based employment according to their preferences and Employment Plan.
• Assess and document needs for support/training and provide on-site training.
• Provide case management and coordinate services among internal and external stakeholders.
• Develop and coordinate a support plan highlighting employment strengths for independence.
• Market services to employers and referral sources to obtain placements and related services.
• Complete intake and annual paperwork to meet billing requirements.
