Responsibilities

• Support individuals in daily living, community activities and socialization.
• Assist with assessments and ISP goals, tracking progress.
• Provide transportation to appointments and events as needed.
• Encourage participation in community activities of choice.
• Assist with accessing health care providers and managing medications.
