Helpful Hearts Health Services Inc. in Baltimore, MD is seeking a Direct Support Professional (DSP) to provide Community Development Services.

You will support individuals in engaging with their communities, developing life skills, and achieving personal goals in a structured, person-centered manner. This role emphasizes compassionate, patient care, safety, and accurate documentation, with a Monday–Friday schedule from 8:00 AM to 2:00 PM.

Qualifications

• High school diploma or GED required.

Responsibilities

• Facilitate community-based activities that promote independence and social engagement.
• Support individuals in developing communication, self-advocacy, employment readiness, and independent living skills.
• Encourage and assist with volunteering, social groups, and recreational activities.
• Implement individualized service plans (ISPs) to help individuals reach their goals.
• Provide transportation to and from community activities as needed.
• Ensure the safety, dignity, and well-being of all individuals supported.
• Maintain accurate documentation and progress notes.
