Job summary

Clarity Care Inc invites applications for the Activities Coordinator role based in Oshkosh, WI. You will oversee Oshkosh Service programming, support participants and staff, and lead the implementation of Chance events.

The position requires coordinating with teams to achieve program objectives in HCBS-compliant settings. The role emphasizes compliance, staff development, risk management, and delivering exceptional customer service across diverse activities.

Qualifications

• Bachelor degree preferred in Human Services or related field; HS diploma considered with relevant experience.
• Three years of direct care and Adult Day Service management experience preferred.
• Proficiency in Microsoft Office (Excel, Outlook, Word).
• Excellent communication and strong organizational skills.
• Must be at least 21 years old.
• Must have a valid Wisconsin driver’s license and meet driving standards; provide auto insurance if required.
• Must pass WI caregiver background check, including DOJ criminal records.

Responsibilities

• Ensure programs operate in compliance with laws, regulations, MCO contracts, and policy.
• Create and implement oversight systems; evaluate monthly metrics and take corrective actions.
• Assess staff development needs and coordinate training resources.
• Monitor safety inspections and address hazards.
• Oversee financial management of Day Service programming with proper record-keeping.
• Monitor vacancy and occupancy to meet goals.
• Ensure HCBS settings compliance and perform self-audits.
• Assist with transporting participants to Day Service events.
• Plan and facilitate events to meet diverse needs and interests.
• Arrange Chance event activities (logistics, food, decorations, staff, volunteers).
• Provide input for long-term sustainability and event ideas.
• Research customer interests and maintain event registrations to ensure cohesive groups.
• Deliver safe, memorable customer service and be prepared for emergencies.
