Job summary

Bright Harbor Healthcare is seeking a full-time SHARE Program Clinician in Toms River to deliver community-based clinical services. You will conduct assessments, develop individualized rehabilitation plans, and supervise case managers to support clients in independent living and recovery.

The role includes providing education to client systems, coordinating referrals, and offering crisis intervention when needed.

Qualifications

Graduation from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling.
LAC/LSW required.
LPC/LCSW preferred.
One year of experience in the psychiatric/mental health field.
Valid N.J. driver's license with less than 6 points.

Responsibilities

Perform therapeutic rehabilitative skill development
Maintain accurate documentation and client records
Recommend referral services as needed
Develop, implement and maintain client IRP
Provide education to client systems related to nutrition, symptom management, medication safety and management, and discharge planning
Provide transportation to clients as needed
Provide crisis intervention services as needed
