Bright Harbor seeks a SHARE Program Clinician to provide community-based assessment and rehabilitation planning for individuals with serious mental illness. You will develop IRPs, supervise case managers, and deliver clinical oversight to support consumers in their home and community settings.

Qualifications

• Graduation from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling.
• LAC/LSW required.
• One year of experience in the psychiatric/mental health field.
• LPC/LCSW preferred.
• New Jersey driver’s license with less than 6 points.

Responsibilities

• Perform therapeutic rehabilitative skill development
• Maintain accurate documentation and client records
• Recommend referral services as needed
• Develop, implement and maintain client IRP
• Provide education to client systems related to nutrition, symptom management, medication safety and management, and discharge planning
• Provide transportation to clients as needed
• Provide crisis intervention services as needed

Job description

SHARE (Supportive Housing Assistance to Reach Excellence) is a supporting housing service designed to assist persons with serious mental illness(es) in maintaining independent housing in the community. The case managers provide the support needed in each person's home environment and in the community. SHARE provides outreach and case management services to program participants. The clinician works with these individuals in the community to complete assessments and develop an Individualized Rehabilitation Plan (IRP). Additionally, the clinician provides clinical oversight for the case managers to support consumers.
