Job summary

Bright Harbor Healthcare is seeking a full-time SHARE Program Clinician in Toms River to deliver community-based clinical services. You will conduct assessments, develop individualized rehabilitation plans, and supervise case managers to support clients in independent living and recovery.

The role includes providing education to client systems, coordinating referrals, and offering crisis intervention when needed.

Qualifications

• Graduation from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling.
• LAC/LSW required.
• LPC/LCSW preferred.
• One year of experience in the psychiatric/mental health field.
• Valid N.J. driver's license with less than 6 points.

Responsibilities

• Perform therapeutic rehabilitative skill development
• Maintain accurate documentation and client records
• Recommend referral services as needed
• Develop, implement and maintain client IRP
• Provide education to client systems related to nutrition, symptom management, medication safety and management, and discharge planning
• Provide transportation to clients as needed
• Provide crisis intervention services as needed

Education

Bachelor's/Advanced degree in Social Work or related field

LAC/LSW required

LPC/LCSW preferred

One year of experience in psychiatric/mental health

Valid New Jersey driver's license with <6 points

Job description

SHARE (Supportive Housing Assistance to Reach Excellence) is a supporting housing service designed to assist persons with serious mental illness(es) in maintaining independent housing in the community. The case managers provide the support needed in each person's home environment and in the community. SHARE provides outreach and case management services to program participants. The clinician works with these individuals in the community to complete assessments and develop an Individualized Rehabilitation Plan (IRP). Additionally, the clinician provides clinical oversight for the case managers to support consumers.
