Job summary

Bright Harbor in Toms River, NJ is seeking a Community Integration Coordinator to support recovery, stability, and independence by connecting clients with community resources. You will develop individualized integration plans and supervise program staff to ensure high-quality care.

The role requires a Master's degree in Rehabilitation, Health and Human Services or related field, plus hands-on experience with mental health services and outreach. Valid NJ driver license required.

Qualifications

• Master's degree in Rehabilitation, Health and Human Services or related field.
• Two years of experience working with individuals with severe persistent mental illness.
• Experience in community outreach, social services, or related area.
• Strong interpersonal and communication skills.
• Knowledge of mental health and community resources preferred.
• Valid NJ Driver’s License with less than 6 points.

Responsibilities

• Provide clinical and administrative supervision to program staff
• Provide ongoing comprehensive assessments including biopsychosocials and substance use
• Actively participate in performance improvement activities for optimal client care
• Participate in Utilization Review to improve strategies and operations for the benefit of consumer care
• Direct and coordinate housing placement by maintaining working relationships with housing providers
• Maintain relationships with adult education programs and coordinate services for consumers
• Provide crisis intervention as needed
• Supervise discharge planning and referral services.
• Participate in personnel actions including recruitment
• Provide direct clinical care and case management services to clients and their families
