Job summary

Georgia HOPE, part of the Health Connect America family, seeks a Client Support Specialist to deliver home- and community-based services. You will work directly with individuals and families to build skills, access resources, and move toward independence, with a focus on trauma-informed care.

Qualifications include a high school diploma or a bachelor’s in behavioral health, plus experience with mental health or behavioral needs. A valid driver’s license is required.

Qualifications

• High School Diploma or Bachelor’s degree in a behavioral health field.
• Experience with children, families, or adults with mental/behavioral needs preferred.
• Valid driver’s license and reliable transportation for community work.

Responsibilities

• Deliver community-based support in clients' homes and local settings.
• Maintain regular contact with clients to monitor needs and progress.
• Review history and plans to align services with goals.
• Help families develop goals in their IRP and teach daily living skills.
• Collaborate with therapists and teams to ensure coordinated care.
• Document service activity in the EMR system and participate in meetings.
