Job summary

REM Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. Social Workers evaluate and treat clients admitted for treatment, using home and community-based settings to support recovery and progress toward goals.

Qualifications

• Master’s Degree in Social Work from an accredited school of social work.
• Membership in NASW and related clinical experience in rehabilitation focusing on traumatic brain injury and spinal cord injury.

Responsibilities

• Evaluation including data gathering from client records, interviews and observations.
• Development and implementation of treatment plans per physician orders with goals and progress evaluations.
• Timely preparation of written documentation of client progress and discharge summaries.
• One-to-one counseling with clients and/or family in home or other settings.
• Increase awareness of brain injury to support recovery and adjustment.
• Attend team conferences and report on client progress and status.
• Complete and submit billing information promptly.
• Communicate effectively with internal case managers.
• Perform other related duties as required.
